In a recent Cardiology article, John Lisko, MD, MPH, Fellow in Training at the Emory Structural Heart and Valve Center, discusses how in an effort to improve outcomes in patients without suitable femoral arterial access, alternative access strategies have been developed.

He notes that transcaval TAVR (first performed in 2013) remains the only alternate access strategy that is completely femoral and not limited by diffuse iliac calcification, retains transfemoral ergonomics, does not increase operator radiation exposure, and is transferrable to other interventional procedures.
